Edited 6/18/25 to include medications authorization form now required by the state of CT.
UPDATED 6/2/25: IF your camper has prepared forms for your camper’s school that include a health asssessment, dated examination AND immunization records - you can use THESE. You do NOT have to fill in and have signed page 2 of the Health Assessment. You should still provide page 1 of this form with a parent signature. Any questions? ann@vivofencingclub.com
Does your camper use OTC or prescribed medications? You will need to fill out the authoirzation form, sign it and have the doctor sign )or e-sign it as well. AND the State of CT wants ONLY ONE drug per form - so each drug gets a separate form.
